COMMUNITY FOCUS NDIS PSYCHOSOCIAL SUPPORT COORDINATION

Community Focus Association is a specialist organisation we believe our strength lies in providing responses that sit with our skill base, We identify as a lived experience practice providing Psychosocial Support Coordination for individuals who experience Mental Health and Addiction, Autism, and Acquired Brain Injury (ABI).

We have, in our 20 years of providing services to the Sunshine Coast, continue to compile an extensive knowledge of mainstream services and community supports to assist choice.

Community Focus Association are a NDIS Registered Psychosocial Support Coordination service.

THE BENEFITS OF PSYCHOSOCIAL SUPPORT COORDINATION

Our goal is to work responsibility with you to assist you to create the most purposefully way to utilise your support budgets to achieve your goals.

Psychosocial Support Coordination is an essential component of developing access to services tailored to meet your needs, support capacity building, inform choices and build on your ability to act independently. We will work with you to interpret and apply your NDIS plan to progress your journey.

Our role is to support you to efficiently and effectively get your choice of services operating at the pace you set, within the parameters of your funded plan.

If you would like us to be your Psychosocial Support Coordinator, you can nominate Community Focus as your preferred Psychosocial Support Coordinator to your planner and they will forward us a "Request for Service" and we will contact you, or you can contact us
Service Providers
  • Assist you to identify the service and service providers that best meet your individual needs, including assessing a number of mainstream, and community provider options
  • Negotiate services to be provided, develop service agreements and  create service bookings with preferred providers
  • Negotiate services and prices as part of any quotable supports
  • Arrange any assessments required
Plan Support
  • Monitor your NDIS plan outcomes/spending
  • Redevelop your goals prior to your NDIS plan review
  • Decide the budget for each support type and advise any relevant plan manager of the breakdown of funds and liaise with any plan manager to establish the appropriate claim categories and attribute the correct amount of funds
  • Provide the NDIS with reports on outcomes and success indicators within the agreed reporting frequency

Arrange meeting to discuss with us what you are looking for in a service
provider,
  • Are we the best service for you
  • What are your options, assist informed choice
  • Do we have the knowledge to effectively link you to appropriate providers
  • We are happy to provide information on other providers if we are not able to effectively meet your needs

Step 2

If you decide to engage us as your Psychosocial Support Coordinator
  • We will provide you with a service agreement pack
  • You will be supported to read and sign agreement
  • Open discussion to clarify any/all elements of agreement
  • A copy will be provided to you at confirmation of agreement
  • Notify NDIS, permission for us to access your plan on PRODA
  • Where required provide information on Plan managers, or if plan manager already engaged provide details.

  • Offer details of available providers
  • Develop timetable with participant
